Full payment is required upon receipt of our billing statement. You are responsible for all non-covered charges. ![]() Co-payments, coinsurances, and/or deductibles, if required by your insurance company, are due before the day of your procedure. Please be sure to bring your insurance cards and a form of identification on the day of your surgery. For your convenience, we will submit all insurance claim forms for covered procedures. Please contact your insurance company to determine if we are a participating provider and what will be covered under your health insurance policy. For the safety of our patients Cornerstone Surgical Facility requires that all paperwork i.e.: consents, lab work, EKG, X-rays and additional tests that your Doctor ordered be signed and be present at the facility the morning of surgery.The Cornerstone Surgery Center fee will vary according to your insurance policy and the type of procedure you are having done.Please arrive one hour before your scheduled appointment.Bring all insurance cards and driver's license with you to the center.If the patient is under 18, a parent or legal guardian must accompany the patient and sign the consent form.We cannot be responsible for damaged or lost property. Leave all valuables, including jewelry and cash, at home.Wear loose, comfortable clothing to accommodate a large bandage after surgery.A plastic container will be provided for their safe keeping. If you wear dentures, you will be required to remove them immediately prior to surgery. Also, remove all makeup, hairpieces, jewelry, and nail polish. This can minimize the chance of infection. You cannot drive yourself or be left alone, nor can you take public transportation unless accompanied by an adult. It is your responsibility to arrange in advance for a responsible adult to drive you home and remain with you the first 24 hours after surgery.If you are diabetic or on blood pressure or heart medication, obtain specific instructions from your physician or contact the center. Please DO NOT take any medications after midnight unless instructed by your physician or our office.Bring a list of your medications with you on the day of surgery, both prescription and non-prescription.Failure to follow these instructions may result in cancelation of your surgery. The staff as well as the doctor are so ridiculously nice, you feel like a celebrity. This will reduce the possibility of nausea and vomiting following anesthesia. Any procedure I have had done here has left me with excellent results. DO NOT drink or eat anything after midnight, including coffee or water.
